01 · Healthcare Interoperability

Your EHR, your lab and your payer all speak different dialects. We interpret — in production, at volume.

Trustbit's healthcare practice approaches hospital systems the way we build ERP: read the live traffic first, write the mapping second, and stay on the roster after go-live. The practice is led by an engineer with 15 years inside hospital integration — EHR feeds, engine migrations, payer EDI, ABDM — backed by the same Trustbit bench that runs plants on 99.9% SLAs.

02 · The exchange, live

What an interface actually does all day.

Four scenarios our practice lead has built over and over across 15 years — a hospital go-live, an ABDM onboarding, a payer claims loop, a lab network. The traffic below is simulated; the discipline is not. Watch the messages move — that's the whole job. Keeping them moving is the career.

03 · What we integrate

Five groups of work. One practice across all of them.

Every engagement below is run by the same team, on the same discipline: live message samples before specs, mappings under version control, a parallel run before every cutover, and a dead-letter queue that never swallows a result silently.

/ 01 · EHR & CLINICAL SYSTEMS

EHR integration — through the doors the vendor actually opens

Epic, Oracle Health (Cerner), athenahealth or the HIS your hospital already runs. We connect apps and platforms through each vendor's sanctioned surfaces — FHIR APIs, SMART launches, HL7 interfaces — and we say plainly what each door does and doesn't allow.

/ sys_01
Epic
SMART on FHIR in-chart launches, US Core reads, orders and results interfaces — scoped against what your Epic org has switched on, not the brochure.
/ sys_02
Oracle Health / Cerner
FHIR R4 endpoints and classic HL7 feeds. We plan for the parts the brochure skips — ops jobs, site-specific field usage, real go-live queues.
/ sys_03
athenahealth
API-first ambulatory integration: appointments, clinicals, claims-adjacent data — built to clear athena's marketplace certification path.
/ sys_04
Any HIS / EMR
Vendor-agnostic connectivity for Indian HIS and specialty EMRs — even the ones that have never shipped an HL7 interface. Especially those.
/ 02 · STANDARDS & DATA PLATFORMS

Standards work — v2 to FHIR, engine to cloud

The pipe-delimited workhorse and the RESTful future, bridged properly. We build and rescue Mirth Connect estates, stand up FHIR facades and data stores, and take Indian providers through ABDM milestone by milestone.

HL7 v2 interfaces
ADT, ORM/ORU, SIU over MLLP — application ACKs, not just accept ACKs. Half the job is Z-segments; we read live samples before we write a line.
/ std_02
FHIR R4 builds
Facade or repository — we'll tell you which and why. US Core or ABDM profiles, SMART v2 auth, bulk $export, subscriptions. Watched at p95, at real cohort sizes.
/ std_03
Mirth Connect estates
Channel development, rationalization and upgrades — including the 4.6+ licensing shift and Java 17 moves — with configs in Git and rollback plans in writing.
/ std_04
ABDM integration
ABHA linking, HIP/HIU flows, consent-manager wiring, sandbox-to-production milestones. The hard part is consent expiry at a busy registration desk — we build for that.
/ std_05
C-CDA & documents
Parse, validate and de-duplicate the XML document family that still carries referrals and discharge summaries between systems.
/ std_06
openEHR & data stores
Archetype-based clinical platforms and queryable FHIR repositories — data you can ask questions of, not a write-only compliance layer.
/ 03 · LABS · DEVICES · PHARMACY

Diagnostics and dispensing — wired to the chart

Orders out, results back, no result ever lost between the analyzer and the chart. LIS interfaces, bedside device connectivity and pharmacy flows — the messages where a silent failure is a clinical event, not a ticket.

Your local test codes mapped to LOINC, your problem lists to SNOMED CT — the unglamorous work that makes every downstream integration honest.
/ 04 · PAYERS & REVENUE CYCLE

Payer rails — claims, eligibility, prior auth

The money-side messages: X12 EDI through clearinghouses in the US, NHCX claims exchange in India, and the FHIR prior-auth stack that CMS-0057-F turns into a hard deadline. Revenue leakage usually traces back to an interface — we go find it.

RCM system feeds
Charge, claim-status and remittance data flowing between your HIS, billing platform and analytics — so denials get caught as data, not as month-end surprises.

NHCX (India)
Eligibility, pre-auth and claim submission on India's national claims exchange — FHIR bundles, consent-bound, built alongside your ABDM stack.
/ 05 · OPERATE & GOVERN

Run it, watch it, own the 2 a.m. page

An interface nobody watches is an outage with a delay on it. We monitor queues, latency and ACK rates around the clock, migrate clinical data without losing history, and keep patient identity and consent honest across every connected system.

Integration monitoring & support
NOC-style watch on every channel — dead-letter queues with replay tooling, alert thresholds, monthly SLA and volume reports. A failed result should page us, not a clinician.
/ ops_02
Healthcare data migration
EHR-to-EHR and legacy archival with record-level reconciliation counts, terminology normalized in flight, and retention obligations met after the old license dies.

05 · The practice

Fifteen years of hospital integration. Three years of Trustbit discipline.

Our healthcare practice lead has spent 15 years inside hospital IT — EHR go-lives, interface-engine migrations, lab networks, payer EDI, and lately ABDM — before joining Trustbit. The company around him is three years old and honest about it: what we bring is his depth, our bench, and the same SLA culture that answers a plant's 11 p.m. call.

An accept ACK only proves the message landed in a queue — not that the result filed to the chart. Interfaces that pass with ten test messages and fall over at ten messages a second are not done. That's the standard this practice is built on.”

What's clinical stays yours

What's clinically appropriate, who may see what, which policies apply — always your clinicians and your CISO. We implement; we don't overrule.

No patient data in sales

Scoping calls run on message structures and anonymized samples. Live PHI only ever moves under a signed agreement and your access controls.

06 · How we engage

Four steps, each with an artifact you keep.

No open-ended discovery. Every step ends in a document your team owns — so even if we stopped tomorrow, you'd be further ahead than when we started.

Interface inventory

We read your live message samples and channel configs — not just the specs. Every feed catalogued, every owner named.

Design & mapping

Field-level mapping against vendor companion guides, architecture decision (facade vs store, direct vs engine), written scope with dates.

Week 2 – 4 · mapping spec

Build & prove

Interfaces built under version control, golden-message regression suites, load tests at production-shaped volumes, then a parallel run with automated diffing.
